fix(changelog): make per-commit meta descriptions unique

The SEO audit fails when more than three pages share a description, and same-day single-file commits (three Archived-sources runs and a research note) all rendered as '26 October 2026: 1 file of code, design or docs'. Append the commit short hash to each changelog page description so every page carries a distinct snippet. Verified: build renders and npm run seo:audit reports no problems found.
